There are several lists of required documents within form cit0014:

  • The essential documents required by everyone for their application:

    • Your 2 pieces of identification (one of them with a photo)
    • Your proof of payment (payment receipt, which you need to print after paying your fees if paying online)

What list of supporting documents I need to replace my citizenship certificate?

That depends on your situation. As we explain directly above, the basic documents you need are:

  • Form cit0001 which is your application form. (Go here for more information)
  • Your citizenship photographs.
  • Your 2 pieces of identification (one of them with a photo)
  • Your proof of payment (payment receipt, which you need to print after paying your fees if paying online)

Here are some additional documents that may be needed in certain situations:

  • Form IMM5476
  • Proof of Legal Change of Name documents
  • Request for Change of Sex/Gender form
  • Previous citizenship or naturalization certificates
  • Retention of Canadian Citizenship certificates
  • Canadian Birth Certificate
  • Proof of other nationalities acquired before February 15, 1977
  • Certificate of Registration of Birth Abroad
  • Foreign birth certificate that lists the names of your parents in the case of which at least one parent was a Canadian citizen at the time of your birh.

FAQs

What documents prove Canadian citizenship?

The following citizenship certificate documents are accepted as proof of Canadian citizenship:

  • Citizenship certificates – including older versions
  • Even though they are no longer issued, citizenship cards are still valid.
  • Canadian birth certificates (unless your parents were foreign diplomats)
  • Naturalization certificates – issued before 1947
  • Registration of Birth Abroad certificates – issued between 1947 and 1977
  • Certificates of Retention of citizenship – issued between 1947 and 1977

PLEASE NOTE: A Canadian passport is not strictly considered proof of Canadian citizenship, although they are only issued to citizens. However, they are official travel documents for Canadian citizens.

Supporting documents have different names on them – what to do?

You will have to provide a legal change of name documentation to unify the name you use on all your documents. That means changing some of your documents to get them under your current name.
